ModerateAntagonism
The administration of sucralfate with enteral nutrition may result in precipitation and the formation of bezoars that may obstruct feeding tubes. The proposed mechanism is the formation of an insoluble aluminum-protein complex between the aluminum in the sucralfate and the protein in the enteral feeding. In addition, sucralfate may not become activated due to the alkaline pH of enteral nutrition products.
What to do: Some experts recommend separating the sucralfate doses from enteral feedings by at least 1 hour. Alternatively, consideration may be given to proton pump inhibitors or H2 antagonists.

Conditions
Conditions in which Sucralfate needs care, as documented in this source.
Hypophosphatemia
MajorSucralfate has phosphate binding properties for which the drug is sometimes used therapeutically. However, hypophosphatemia may occur in some patients, regardless of renal status. Therapy with sucralfate should be administered cautiously in patients with preexisting hypophosphatemia. Monitoring phosphate levels in these patients is recommended.

Kidney Diseases
MajorPatients with chronic renal failure or those receiving dialysis have impaired excretion of absorbed aluminum. Additionally, aluminum does not cross dialysis membranes as it is bound to albumin and transferrin plasma proteins. Aluminum accumulation and toxicity (aluminum osteodystrophy, osteomalacia, encephalopathy), have been described in patients with renal impairment. Sucralfate must be used with caution in patients with chronic renal failure.
